A good dental referral form collects what the specialist needs before the patient ever calls: who they are, why you are sending them, which teeth, their medical history, and the X-rays. Build it once as an online form, send the link to your orthodontist or oral surgeon's office, and most of the back-and-forth phone calls stop.
Start from the intake template
Why referrals arrive incomplete
Most incomplete referrals start as a handwritten slip, a fax cover sheet, or a WhatsApp message woth a photo of an X-ray. Nobody forgot on purpose. The sender just didn't know what the specialist's front desk would ask first, so the specialist's office calls you for the date of bitrh, calls again for the insurance, and the patient waits.
A form fixes this because required fileds can't be skipped. You agree on the list once, and every referral follows it. Before you build anything, ask your orthodontist and your oral surgeon for the last five things they had to phone you about. That list is your form.
The fields to include
This is the core set. Mark the first group required and the rest as needed for your specialists.
| Field | Block type | Why the specialist needs it |
|---|---|---|
| Patient full name | Short answer | Matching the referral to the person who calls |
| Date of birth | Date | Identifies the patient and tells them if it's a minor |
| Parent or guardian name and phone | Short answer, shown only for minors | Consent and scheduling |
| Patient phone and email | Phone number, Email | Booking without calling you |
| Insurance provider and member ID | Short answer | Eligibility checks before the visit |
| Referring dentist, practice, phone, email | Short answer | Where to send the report back |
| Specialist | Dropdown | Drives the extra questions below |
| Reason for referral | Long answer | The actual question you want answered |
| Tooth numbers or area | Short answer | Saves them hunting through films |
| Urgency | Multiple choice (routine, soon, urgent) | Decides who gets a call today |
| Medical history, medications, allergies | Long answer | Safety before any procedure |
| Blood thinners | Multiple choice (yes, no, unsure) | Matters for surgery |
| Treatment already done | Long answer | Avoids repeating work |
| X-rays and photos | File upload | The thing most referrals are missing |
| Patient knows about the referral | Checkbox | Fewer surprised patients on the phone |
| Consent to share records | Checkbox | A written record that you asked |
Keep wording plain. "Tooth numbers (e.g. 17, 18)" gets better answers than "Area of concern."
Add questions only the right specialist needs
An orthodontist and an oral surgeon don't ask the same things, and one long form for both gets ignored. Use conditional logic so the Specialist dropdown shows or hides sections. The conditional logic help page walks through it.
For an orthodontic referral, you might show:
- The patient's main concern (crowding, spacing, bite)
- Whether permanent teeth have come in
- Any previous braces or aligners
- Front and side photos
For an oral surgery referral, you might show:
- The procedure you're asking about (extraction, impacted tooth, implant, other)
- Whether the patient wants sedation
- Any bleeding or healing problems you know of
- Panoramic X-ray or scan
Your specialists know their own intake better than any template does. Show them the draft and let them cut or add.
Build it in about 20 minutes
Qwenvo is a free form builder that our team makes, and it works well for this. Here's the order I'd follow.
- On the empty-form screen, describe the form in a sentence ("dental referral form to an orthodontist or oral surgeon") and let the AI draft it. If you already have a paper referral form, import the Word file, PDF, or a screenshot of it instead.
- Fix the fields against the table above. Type
/to add a block. - Add the logic for each specialist.
- Open Settings. Turn on email notifications so each referral lands in your inbox, and turn on the confirmation email so the sender gets a copy. Switch off the Qwenvo badge so it looks like your clinic's form.
- On the Share tab, copy the public link. You can paste it into WhatsApp or email, or embed it on your clinic site. If you want a tidy addrezs like referrals.yourclinic.com, add it on the Domains page.
Several things that Tally locks behind its $29/month Pro plan are free on Qwenvo: removing the badge, a custom domain, teammates (so your front desk can have a login), partial submissions, visit and drop-off analytics, and custom design and CSS. There are also 180+ templates and AI triage, which can tag each submission with a priority such as urgent, so a same-day case doesn't sit in the pile.
Tally's free plan is still a solid choice if you only need a simple form. Its free file uploads are limited to 10 MB per file, which is tight for scans. Qwenvo's file upload takes files up to 1 GB each.
Read this before you collect patient data
A referral contains health information, so this part matters more than the design.
Qwenvo has no HIPAA offering, and it is hosted in the United States. If your clinic has to meet HIPAA, Jotform lists HIPAA features on its paid plans. Check with wohever handles compliance for your practice before you pick any tool.
Google Forms is free with a Google account, but respondents must sign in with Google to upload files, and design options are limited. That can trip up a specialist's staff who use a different email system.
If you are unsure, a cautious middle path is to collect the contact details and the reason for the referral in the form, and send X-rays the way your practice already shares patient records. In every case, keep the consent checkbox and set the form to delete responses after a set number of days (Qwenvo's data retention setting does this).
Keep the loop closed
A referral isn't finished when it's sent. Once a week, open the Submissions table and check which referrals have no report back. Export to CSV if you want a running list in a spreadsheet. If your front desk forwards referrals to the specialist, the per-submission PDF is the quickest thing to attach.
Questions owners ask
Should my team fill it out or the patient?
Your team, because they know the tooth numbers and the clinical reason. If you want patients to enter their own insurance and contact details, make a second short form for that and link the two.
Can I attach X-rays?
Yes. Add a File upload block. On Qwenvo, each file can be up to 1 GB. On Tally's free plan the limit is 10 MB per file, and on Google Forms the sender has to sign in with Google.
Do I need a website to use this?
No. The public link works in WhatsApp, emil, or a text message. Qwenvo has no built-in QR code, but you can paste the link into any QR generator and print it for the front desk.
Is it really free?
Qwenvo costs $0, with no paid plans, no card, and no trial. A fair use policy applies if an account reaches around 50,000 submissions or 50,000 emails a month, which a dental practice won't come near.
This week, call the two specialists you refer to most and ask for the last five things they phoned you about. Type those into a new form and send them the link to review.
