A pickup and delivery order form for a laundry shop needs six things: name and phone, address, item types with quantities, a pickup slot, special care notes, and a return date. You can build one free in Qwenvo in one sitting, share the link on WhatsApp, and stop retyping orders from voice notes.
The fields a laundry pickup form needs
Look at your last five WhatsApp orders. Most of them needed a follow-up: which flat, how many pieces, what time works? The form asks all of that once, in the order you need it.
| Field | Block | What to put in it |
|---|---|---|
| Name | Short answer | First name is enough |
| Phone number | Phone number | The rider calls this from the gate |
| Pickup address | Long answer | Flat, floor, landmark |
| Service | Multiple choice | Wash and fold, wash and iron, dry clean, iron only |
| Items | Number (one per item type) | Shirts, trousers, sarees, bedsheets, blankets, suits |
| Pickup date | Date | Today onward |
| Pickup slot | Dropdown | The windows your rider actually runs |
| Special care notes | Long answer | "Silk saree, no starch. Stain on left cuff." |
| Stain or damage photo | File upload | Optional |
Keep the item list to what you take every week. Add an "Anything else" long answer for the rest, so a customer with a curtain to clean isn't stuck.

Pickup slots and return dates
Make the slot a dropdown with fixed windows, for example 9 to 11 am, 11 am to 1 pm, and 4 to 6 pm. Qwenvo has no built-in scheduling, so the slot is a request until you confirm it. Say that on the thank-you page: "We'll message you to confirm your slot."
For the return date, put your turnaround in a Text block. Something like "Wash and fold comes back in 2 days, dry clean in 4" (use your own numbers). Then ask "When do you need it back?" with two choices: standard turnaround, or sooner. Use conditional logic to show a Date question only when they pick "sooner". Most customers skip it, and the wedding-outfit customer gets to tell you.
If your prices are fixed, calculated field can total the order as the customer fills it in. Say shirts are ₹30 each and bedsheets ₹80. Label the total "Estimate", because you confirm it after counting at pickup.
Build it in Qwenvo
Qwenvo is a completely free form builder that our team makes. It has an order form template you can start from, and our guide to a free order form builder covers the general case.
- Open the order form template and rename it with your shop name. If you'd rather start from a description, type something like "pickup and delivery form for a laundry with item counts, pickup slot, care notes and return date" on the empty-form screen. Inside a form, Ctrl J opens Ask Qwenvo for chanbes.
- Type
/to add the blocks from the table above. Type@to reuse an earlier answer, such as the name, further down the form. - Open Customize and add your logo, cover image and shop colors.
- In Settings, turn on email notifications to you and switch off the Qwenvo branding toggle so the form looks like your shop. Turn on partial submissions too.
- On the Share tab, copy the public link and set the liink preview image, title and description. That is what shows up when the lnik lands in a WhatsApp chat.
Several things that many form builders charge for are free here: removing the badge, a custom domain, teammates, partial submissions, visit and drop-off analutics, AI triage, custom design and CSS, and 180+ templates. For a laundry, the ones you will use first are the clean branding, the partial submissions and your own subdomain.
Share the link and handle incoming orders
Paste the link into your WhatsApp status and customer chats, your Instagram bio, and the website field on you're Google Business Profile. If you own a domain, add a subdomain like forms.yourlaundry.com: add a CNAME pointing to qwenvo.com plus the TXT record shown on the Domains page, click Verify, then pick it on the Share tab. It has to be a subdomain, because a bare domain usually can't take a CNAME.
There is no built-in QR code. Paste your link into any QR generator and print the result on the bag tag and the counter.
Once orders arrive:
- Each submission emails you, and all of them sit in the Submissions table. Export CSV or XLSX for the rider's route sheet.
- You can add unlimited teammates, so the counter staff and the rider see the same orders.
- Partial submissions show up as incomplete rows. Someone who typed an address and stopped is worth a quick call.
- Drop-off analytics shows which question loses people. If many leave at the photo upload, make it optional or move it to the end.
- AI triage gives each order a priority and a one or two sentence summary. Routing rules can send high priority ones to up to 5 email addresses.
Other tools worth a look
Google Forms is free with a Google account and fine for a plain order list. Design and branding are limited, it has no built-in payments, and customers must sign in with Google to upload a stain photo. That last one will lose you orders.
Tally has a free plan with unlimited forms and submissions, payments, and file uploads up to 10 MB per file. Removing its branding, a custom domain and partial submissions are on Pro at $29 a month. If you take payments through the form, Tally is cheaper: it adsd no fee on top of Stripe. Qwenvo pays out through PayPal only, and keeps 1% on top of PayPal's own fee. Most laundries collect cash or transfer at the door anyway, so you may never need the Payment block.
Questions owners ask
Can customers pick a slot, and will it stop double booking?
The dropdown records the customer's choice but doesn't reserve it, since Qwenvo has no built-in scheduling. You confirm each slot by message. If you want real calendar booking, embed Calendly inside the form with Embed anything.
Do I need a website?
No. The form has its own link and opens in any phone browser. A custom domain is optional.
Will it work in Hindi or another language?
Write your questions in whatever language your customers use. The Settings drawer also has a language option covvering 45+ languages, including right-to-left ones. One catch: per-submission PDF slips can't draw some scripts, including Devanagari. CSV and XLSX exports handle them fine, so print your route sheet from those.
How do I see orders if I'm not at a computer?
Qwenvo has no mobile app, but the Submissions table opens in your phone browser, and new orders arrive as email notifications. Give your rider's phone the same access through a teammate account.
Send it to three customers first
Open the order form template tonight, swap in your own items and prices, and send the link to three regular customers. Notice what they still ask you on WhatsApp afterward, fix that field, and only then put the link on your Google Business Profile.
