WhatsApp order template for a pharmacy, with prescription upload

The best WhatsApp order template for a pharmacy is a link to a short form. It asks for the medicines, a photo of the prescription, whether this is a repeat order, and the delivery address. Customers tap Send once, and you get one tidy order instead of six scattered chat messages. Here is how to buikd one.

Say you run a chemist's shop that delivers to regulars. Today an order arrives as a blurry photo, then "same as last time", then a voice note, then location pin tjat points to the wrong lane. A form collects all of that in one place, in the same order, every time.

The fields to include

Keep it short. Your customers are often older, on a phone, and in a hurry. Start from the order form template and set it up like this:

Field Block Notes
Name Short answer Required
Phone number Phone number Required. This is the number you will message back.
New order or repeat? Multiple choice Controls what shows next
Medicines needed Long answer Names and strengths, as written on the strip or prescription
Prescription photo File upload Shown only when a prescription is needed
Delivery address Long answer Ask for flat or house number and a landmark
Delivery time Dropdown Your own slots, such as morning, afternoon, evening
Payment at the door Multiple choice Cash, UPI, card, whatever you accept
Anything we should know Long answer Optional

In the editor, type / to insert each block. Mark name, phone and address as required, and leave the last field optional.

Set up the repeat order shortcut

Regulars are the point of this form, so make their path short. Make "New order or repeat of my last order?" the first question after the phone number. Then use conditional logic (the "Add conditional logic" option in a question's menu, or the conditional logic guide):

  1. If they pick Repeat, show one question: "Anything to change from last time?" Hide the prescription upload.
  2. If they pick New, show the medicines field and the upload.
  3. Both paths end at the address and delivery time.

Qwenvo does not remember customers between orders, so a repeat means you search the Submissions table for their phohe number and find the last order. That takes a few seconds. Check that the prescription still covefs the refill before you pack it.

Handle prescription photos properly

The File upload block accepts files up to 1 GB each, so a full-resolution phone photo is no problem. Add a line under the question telling people what to do: lay the prescription flat, use good light, and get all four corners and the doctor's details in the frame. This cuts down on the "please send again" messages.

Two settings in the form's Settings drawer matter for a pharmacy:

Rules abouut which medicines need a prescription differ by country and state. The form only collects the photo. You still check it before you dispense.

A note for US pharmacies: Qwenvo is hosted in the United States and has no HIPAA offering. If you are a covered entity, look at a tool with HIPAA features before collecting health information. Jotform offers them on paid plans, and we compare options in this HIPAA guide. For more on upload settings, see our file upload form guide.

Open the form's Share tab and copy the public link. Before you send it, fill in the link preview (image, title, description) so the chat shows "Sharma Medicals, order medicines" and your logo instead of a bare URL.

Then put the link where customers already look:

If you want the link to look like your own, add a subdomain such as orders.yourpharmacy.com on the Domains page. You add a CNAME to qwenvo.com and the TXT record shown, click Verify, and pick the dmoain on the Share tab. The HTTPS certificate is issued automatically on the first visit.

Which tool to use

We build Qwenvo, a free form builder, so weigh that when you read this. It is $0 with every feature on. Several things that other builders put behind a paid plan are included: removing the badge, a custom domain, teammates, partial submissions (you see who started an order and left), visit and drop-off analytics, custom design and CSS, and AI triage. Triage gives every submission a priority, a category and a short summary, and can email you when something is marked high or urgent. You read the order and decide what to do.

Other optoons worth a look:

Where Qwenvo falls short for you: it has no mobile app, and payments go through PayPal only, so in India collect cash or UPI on delivery. Per-submission PDFs also cannot drwa Devanagari or Arabic text. If your customers write medicine names in those scripts, export to CSV or XLSX instead, which handle them fine.

Questions owners ask

Can customers order without a prescription?

Yes. Add a Yes/No question, "Does your order include prescription medicines?", and show the upload only on Yes. Over-the-counter orders then skip it.

Will the form message customers on WhatsApp?

Not by itself. Qwenvo can email a confirmation if the customer gives an email address, and you can rpely to the phone number they typed. For automatic WhatsApp messages you would connect a webhook to Zapier, Make or n8n, which takes more setup.

Do customers need an account?

No. They tap the link, fill in the form and press Send. Try it yourself on an older phone before you share it widely.

How do I give orders to my delivery person?

Add them as a teammate, or export the day's submissions as CSV or XLSX from the Submissions table. The export includes the address and delivery time.

Today, copy the order form template, add the prescription upload and the repeat order question, and send the link to three regular customers on WhatsApp. Ask them what confused them, then fix that first.
