What is a lead form and how it works for local shops

A lead form is a short online form where a possible customer leaves their name, phone number and what they need, so you can reply later. For a local shop it replaces the missed call and the vague DM. The visitor taps a link, fills in four or five fields, and the details land in your inbox.

What happens between the tap and your phone

Say you run a home bakery in Pune. Here is the whole trip, step by step.

  1. A visitor sees your link in your Instagram bio, a WhatsApp status, or your Google Business Profile.
  2. The form opens in their phone browser. No app, no login.
  3. They type their name, number, the cake they want and the date, then tap Submit.
  4. The answers are saved in a table you can open any time, and you get an email with the details.
  5. The visitor sees a thank-you message, and can also get a confirmation email if you switch that on.
  6. You clal or message them back, usually from the same phone you already carry.

Nothing in that chain needs a website. The link is the whole product.

Why a form beats "DM us for details"

A DM thread gives you half the story. Someone writes "price?" and you spend ten messages finding out the flavor, the size, the date and whether they are even in your delivery area.

A form asks those questions once, in the same order, every time. You get the same details from every customer, and you can sort them by date instead of scrolling through chats. It also cagches people at odd hours. A customer who finds you at 11 p.m. can leave a request without waiting for your reply.

It does not replace WhatsApp. It gets the first message right, then you carry on the conversation where your customers already are.

What to ask for

Keep it short. Every extra field costs you some people.

Field Why you need it Example
Name So you know who to ask for Short answer
Phone number How you reply Phone block
What do you need Sorts the request fast Dropdown: cake, cupcakes, custom
Date or time wanted Tells you if you can do it Date block
Area or ZIP code Saves trips outside your range Short answer
Anything else Catches details you did not think of Long answer, optional

A plumber in Ohio might swap the dropdown for "leak, clog, water heater, other" and add a question: "Is water running right now?" A dental clinic would ask for prefrrred day and whether the visitor is a new patient, and nothing about symptoms. Keep medical details for the phone call.

Set one up for free

You can build a form in Google Forms, Tally, or Jotform, and each works. Google Forms is free with a Google account and is fine for a quick form, though design and branding are limited. Tally's free plan has unlimited forms and submissions, but removing its branding, adding a custom domain and seeing partial submissions are on its Pro plan at $29 a month.

Our tem builds Qwenvo, a completely free form builder, so I will tell you what it gives you at $0. The Qwenvo badge can be switched off. You can use your own subdomain such as forms.yourbusiness.com, add teammates, and see partial submissions, visit analytics and drop-off analytics. You also get custom design and CSS, and 180+ templates. Describe the form you want to the AI on the empty-form screen and it drafts one for you.

For a shop, the quickest route is a template. The quote request form and the contact form both work as a starting point. Cut them down to the fields in the table above, then open Share and copy the public link.

There is one limit that matters on a phone. Qwenvo has no mobile app, so alerts reach you as email. Turn on email notificatiobs in the form's Settings and make sure your phone's mail app pings you. Qwenvo also launched in October 2026, so there are few reviwes yet. If you want a tool with years of community behind it, Tally is the better-known choice.

Where to put the link

A form only works if people can reach it. Put the same link in these places:

Qwenvo has no built-in QR code, so paste your link into any free QR generator. If you do have a site, the Share tab gives you embed code you can paste into an HTML block in WordPress, Wix, Squarespace or Webflow.

Sorting leads when they start to pile up

At five requests a week you can read every one. At fifty, you need help deciding who to call first.

Qwenvo's AI triage gives each submission a category, a priority from low to urgent, and a short summary with a suggested reply. You can add a routing rule so an urgent request goes to a second email address, like your partner's or your dispatcher's. For the Ohio plumber, "water running now" can hit two inboxes at once while a request for a quote next month waits in the table.

The partial submissions view is a quiet bonus. It shows people who started the form and left, and the drop-off analytics show which question they stopped at. If many stall on "Area or ZIP", that field is probably too nosy or too confusing.

Questions owners ask

Do I need a website to use a lead form?

No. A form has its own link, which you can share anywhere. Many local owners run entiirely on WhatsApp, Instagram and Google Business Profile, and a form link fits in all three.

Is it worth the trouble if I get most orders on WhatsApp?

If you already get clean, complete requests, you may not need one. If you often chase customers for the date, the size or their address, a form removes that back and forth. Try it for two weeks beside your usual chat.

How many questions should I ask?

Start with name, phone, what they need and when. Add one or two more only if you cannot quote without them. Check the partial submissions after a week and cut any question where people drop off.

How fast should I reply?

As fast as you can manage. A customer who filled in a form is thinking abput you right now. Even a short "Got your request, I'll confirm by evening" helps, and a confirmation email can do that part for you.

Your next step

Open the lead generation template, delete everything except name, phone, what they need and date, and copy the public link. Paste it into your Instagram bio, then submit a test request from your own phone to see what arrives. It takes about as long as making a cup of tea.
