A request for quote form works when it asks only what changes your price, promises a replly time, and says what happens next. Below is copy you can paste for the form intro, the field labels and the confirmation message, written for a plumber, a painter and a home baker. Swap in your own times and phone number.

What a quote form needs to ask and say
A text that says "how much to paint my flat?" gives you nothing to price. A form asks the same questions every time, so your first reply can already be a number. Keep it to six or seven fields. Every extra one is another reason to quit on a phone keyboard.
| Field label | Block | Why it is there |
|---|---|---|
| Your name | Short answer | So your reply starts with a name |
| Best phone or WhatsApp number | Phone number | Where your quote goes |
| Where is the job? | Short answer | Tells you if it is in your area and what travel costs |
| What do you need done? | Long answer | The main input for your price |
| Photos (optional) | File upload | Can save you a site visit on simple jobs |
| When do you need it? | Multiple choice | Rush jobs cost more |
The intro matters as much as the fields. Say what you need from them, how long it takes, and when they will hear back. This version fits almost any trade:
Tell us about the job and we will send a price within one working day. It takes about two minutes. Photos help us get the number right.
The confirmation message is the last thing people read, so give it a time:
Thank you. We have your request and will contact you on the number you gave by [time]. If it is urgent, call [your number].
Wording for a plumber
Form title: Request a plumbing quote
Intro:
Tell us what is leaking, clogged or broken and we will call you back the same day with a price or a visit time. For a burst pipe or no water at all, call [your number] instead of using this form.
The emergency line keeps a real emergency from sitting unread in your inbox.
Field labels:
- What do you need help with? (Leak, Clog, Water heater, Fixture install, Something else)
- Describe the problem
- Photo of the problem, or of the label on the water heater (optional)
- Address or ZIP code
- Best number to reach you
- When can someone be at the property?
Confirmation:
Got it, thank you. A plumber will call from [your number] by 6 pm today if you sent this before 3 pm. Otherwise we will call first thing tomorrow.
Wording for a painter
Form title: Get a painting quote
Intro:
Send us the rooms, the rough size and a few photos and we will email a written quote within two working days. We price by the room, so a photo of each wall helps a lot.
Field labels:
- Which rooms or areas? (checkboxes: Living room, Bedrooms, Kitchen, Bathroom, Exterior)
- Approximate size (number of rooms or square feet)
- Photos of the walls and ceilings
- Is the space empty or furnished?
- Email address
- Which month do you want to start?
Asking whether the space is empty or furnished is worth the space. It changes prep time, and customers rarely mention it unprompted.
Confirmation:
Thank you. We will look at your photos and email a written quote within two working days. If we need to see the space first, we will suggest a visit time.
Wording for a home baker in Pune
Form title: Custom cake enquiry
Intro:
Tell me the date, how many people you are feeding and the flavor you have in mind. I reply on WhatsApp within a few hours with a price and what is possible. I bake in small batches, so please order at least three days ahead.
Field labels:
- Date needed
- Number of guests or servings
- Flavor and design ideas
- Reference picture (optional)
- Pickup or delivery?
- Delivery area (shown only if they pick delivery)
- Any allergies I should know about?
- WhatsApp number
Confirmation:
Thank you! I have your request and will message you on WhatsApp between 9 am and 8 pm with a price. Your date is not booked until you confirm and pay the advance.
That last sentence prevents the awkward "but I thought you were holdibg my date" conversation.
Putting the wording into a form
The copy works in any builder. If you want a place to put it, Qwenvo is a free form builder that our team makes, and its quote request template already has fields like these.
- Open the template and paste your itnro into the first text block.
- Rename the fields using the lists above.
- Add a thank-you page and paste in your confirmation message.
- In Settings, turn on email notifications to yourself.
- On the Share tab, copy the public link and put it in your Instagram bio, your Google Business Profile and your WhatsApp replies.
The photo field accepts files up to 1 GB each, so a phone video of a leaking pipe goes through. For the baker's delivery area, use conditional logic so the question appears only when someone picks delivery.
Several things Qwenvo gives you free sit on Tally's $29 a month Pro plan: removing the badge, a custom domain such as quotes.yourbusiness.com, teammates, partial submissions, custom design and CSS, and visit and drop-off analytics. AI triage, which gives each request a category, priority and suggested reply, is free too, along with 180+ templates.
Other tools can be the better pick. Tally's free plan has unlimited forms and submissions, with uploads up to 10 MB per file. Google Forms is fine if your form is text only, but customers must sign in with Google to upload a photo, which hurts a quote form. One Qwenvo limit matters in Pune: its per-submission PDFs cannot draw Devanagari, so if customers write in Hindi or Marathi, export to CSV or XLSX instead.

Questions owners ask
How long should a quote request form be?
Six or seven fields is plenty. If you keep asking customers the same follow-up question on the phone, add it to the form. If you never use an answer, delete the field.
Should I ask for a budget?
It helps when your jobs vary widely, such as a painter who does both a signle room and a whole house. Offer ranges as multkple choice. If you quote from photos and a description, you can skip it.
Can customers send photos?
Yes, with a file upload field. Qwenvo accepts up to 1 GB per file, Tally's free plan up to 10 MB, and Google Forms requires a Google sign-in. Mark the field optional so people without photos can still send the request.
How do I find out why people abandon the form?
Turn on partial submissions and check the drop-off analytics. If most people leave at one field, such as the address, reword it or make it optional.
