Free client feedback form for tradespeople and tutors

The best free client feedback form for a plumber, electrician or tutor is a short one: five questions, no login, built to be answered on a phone in under a minute. Send the link by WhatsApp or text the same day you finish the job, and use a free builder so you pay nothing for it.

Five questions that fit a job at someone's home

Generic feedback forms ask about "the experience." Your clients care whether you arrived when you said, whether the bill matched the quote, and whether you left the place tidy. Parents of a tutor's students care about punctuality, fees and progress.

Five questions cover it:

# Block Plumber or electrician Tutor
1 Rating How did the job go overall? How was this month's tutoring overall?
2 Multiple choice Did we arrive when we said we would? Did sessions start and end on time?
3 Multiple choice Was the final bill what we quoted? Is the fee and schedule working for you?
4 Long answer What could we have done better, including how we left the place? What should I change to help your child more?
5 Multiple choice, then Short answer May we share your comments publicly? If yes, what name should we use? Same

Put the long answer fourth, so people are already tapping through by the time they have to type. The last question protects you: you post a comment on Instagram or your Google Business Profile only when the client said yes.

What to send, and when

Mention it before you leave: "I'll send you a one-minute form tonight." The message that arrives later then looks expected.

A plumber in Ohio might send this the same evening:

Hi Mrs. Carter, it's Dan from Dan's Plumbing. Thanks for having me out today. If you have a minute, this short form tells me how the job went: [link]. If anything isn't right, just reply here and I'll come back.

A tutor in Pune could send this on the lat day of the month:

Hi Mr. Kulkarni, this is Meera. Here is a short form about this month's classes. It has five questions and takes about a minute: [link]. Honest answers help me plan next month.

Send job forms the same day, while the work is fresh. Tutors can ask at month end, or after a test or a term. If nobody answers after two or three days, send one gentle reminder and leave it. The "reply here and I'll come back" line gives an unhappy client a quicker route to you than a form does.

Set it up in Qwenvo

I'd build this in Qwenvo, a free form builder our team makes. There are no paid plans, no card and no trial. The customer feedback template gives you a starting layout, so you edit instead of starting blank.

  1. Open the template and swap in the wording from the table. Type / to add a block like Rating or Long answer.
  2. Add your logo and colors in the Customize panel.
  3. In Settings, switch off the Qwenvo branding toggle and turn on email notifications to you.
  4. On the thank-you page, thank the client and add your Google review link, shown to everyone who finisehs the form.
  5. On the Share tab, copy the public link. For an invoice or a van sign, paste it into any QR code generator, since Qwenvo has no built-in QR codes.

Several things most form builders charge for are free here. Removing the badge is one. Partial submissions are another: if a client starts the form and quits, you see an incomplete row instead of nothing. Visit and drop-off analytics show where people stop. Teammates are unlimited, so a spouse or office helper can read replies too. You can also use a subdomain like feedback.yourbusiness.com, and the custom domains guide walks through the DNS records.

If you send one link to everyone, a hidden field can carry the client's name or job inside it, so you never wonder who a reply came from. The hidden fields guide explains how.

Turn on "Triage submissions with AI" in Settings and each reply gets a category, a priority from low to urgent, a sentiment, a one or two sentence summary and a suggested reply. A routing rule can email up to five addressses, so a complaint about a leak lansd in your inbox flagged as urgent. A monthly AI usage allowance applies to each account.

Google Forms, Tally and where Qwenvo falls short

Google Forms is free with a Google account and handles a five-question form without trouble. Its design and branding options are limited, so the form looks like a Google form. If a plain look is fine, it is enough.

Tally's free plan includes unlimited forms and submissions. Removing its branding, a custom domain, partial submissions and visit and drop-off analytics are on Pro, which costs $29 a month ($24 a month if you pay yearly). Tally is built and hosted in Europe, while Qwenvo is hosted in the United States, which may matter to you.

Qwenvo has its own limits. It launched in October 2026, so there are few reviews yet. It has no mobile app, so you read replies in your phone's browser or in the email notifications.

A ten-minute weekly routine

Ten minutes a week is enough once the form is live.

When What to do
Same evening as a job Send the form link by WhatsApp or text
Friday Open the Submissions table, read new replies, and message anyone who rated you low
Friday Share one or two approved comments on Instagram or your Google Business Profile
End of month Check drop-off analytics and shorten any question where people quit

Questions owners ask

Do I need a website to use a feedback form?

No. The form has its own public link, so you can paste it into WhatsApp, a text message or your Instagram bio. If you add a website later, Qwenvo's embed options let you put the same form on any site builder that accepts HTML.

Should I ask for a Google review in the same form?

Put the review link on the thank-you page, after the answers, and show it to everyone instead of only the people who gave top marks. If you would rather send the review ask as its own message, the local business review request template is a starting point.

How many questions are too many?

For a home visit I'd stop at five. If you add more, check the drop-off analytics after a few weeks. They show which question people quit on.

Can I collect feedback in Hindi, Marathi or another language?

Write the questions in whatever language your clients use. The form language setting covers 45+ languages, including right-to-left ones. One catch: Qwenvo's per-submission PDFs cannot draw some scripts, including Devanagari, so use the CSV or Excel export for those replies.

Next step: open the customer feedback template tonight, change the five questions to match your trade, and send the link to the last client you worked for.
