A quick customer feedback form needs four fields: a star rating, one open comment, permission to quote it, and an optional phone number for a reply. It is short enough that customers finish it and simple enough to build in a minute. Copy the sample below, send the link on WhatsApp, and read the answers once a week.

The sample: four fields to copy
Keep the wording short. People answer on their phone while standing at your counter or waiting for a delivery.
| # | Question | Field type | Required |
|---|---|---|---|
| 1 | How was your visit today? | Star rating, 1 to 5 | Yes |
| 2 | What is one thing we should keep doing or fix? | Long answer | No |
| 3 | Can we share your comment on our page? | Multiple choice: Yes / Yes, without my name / No | Yes |
| 4 | Phone number, if you'd like us to reply | Phone number | No |
Change "visit" to "order" if you bake cakes, or to "service call" if you fix pipes. Leave the rest alone.
Title the form "Tell us how we did." On the thank-you page, write one line: "Thanks, we read every answer." Only write that if you will, because customers notice when nobody replies.
Build it in about a minute
I'll use Qwenvo for the steps. Our team builds it, and it is completely free. The sample works the same in any builder.
- Open the customer feedback template, or start a blank form.
- Type
/and add a Rating block, a Long answer block, a Multiple choice block and a Phone number block. - Paste in the quesstions from the table and mark fields 1 and 3 as required.
- Open the Settings drawer and turn on email notiifcations, so each answer lands in your inbox.
- Go to the Share tab and copy the public link.
If you would rather skip the clicking, describe the form on the empty-form screen and let Create with AI draft it. Something like "feedback form for a home bakery in Pune with a star rating, a comment, permission to quote and an optional phone number" is enough. If your customers prefer Hindi, Marathi or Tamil, the language setting covers 45+ languages.

Why these four fields work
The star rating gives you a number you can compare week to week. A bakery that averages 4.2 one week and 4.6 the next can see something changed.
The comment gives you the reason behind the number. Ask for one thing, not a list, and people actually write something.
The quote permission is the field most owners skip, and it is the one that pays you back. A customer who ticks "Yes" has just handed you a testimonial for your Instagram or Google Business Profile.
The phone number lets you fix problems while they are small. If a dental clinic gets a 2 from a patient who left a number, the receptionist can call that afternoon. That call is often worth more than the sale you almost lost.
Get the link in front of customers
A form nobody sees collects nothing. Pick one or two of these and use them every time:
- After the job: a plumber in Ohio can text the link when he sends the invoice.
- In the delivery message: the Pune baker adds the link under the "yoour order is on the way" WhatsApp message.
- At the counter: print a QR code that opens the form.
- On Instagram: put the link in your bio or a story highlight called "Feedback."
Qwenvo has no built-in QR code. Paste your public link into any free QR generator, download the image and print it. We have a walkthrough for a feedback form with a QR code if you want the steps.
What you get free, and what to know
Most form builders charge for the extras. On Qwenvo these are all free: removing the Qwenvo badge, a custom domain, unlimited teammates, partial submissions, visit and drop-off analytics, AI triage, custom design and CSS, and 180+ templates.
For a small business, the useful ones are a clean form with no badge, a link like feedback.yourbakery.com (subdomains only, set up on the Domains page), and drop-off analytics. If people open your form and leave without finishing, the numbers show it.
Other options are fair choices too:
- Tally has a good free plan with unlimited forms and submissions, and this four-field sample fits it fine. Removing branding, custom domains and partial submissions need its Pro plan at $29 a month.
- Google Forms is free with a Google account and the easiest if you already live in Gmail. Design and branding choices are limited.
Two honest limits on our side: Qwenvo launched in October 2026, so there are few reviews of it yet, and it has no mobile app. You build and read answers in a phone browser, which works, but it is not an app on your home screen.
A 15-minute weekly routine
Pick one slot, say Friday afternoon, and do the same five things.
| Step | What to do |
|---|---|
| 1 | Open the Submissions table and read the 1 to 3 star answers first. |
| 2 | Call or WhatsApp anyone who left a number and had a bad experience. |
| 3 | Scan the comments for a word that shows up twice, like "waiting" or "price." |
| 4 | Pick one fix for next week and tell your staff. |
| 5 | Copy one "Yes" comment to Instagram or your Google Business Profile. |
Check the Insights tab for the average rating, and compare it with last week's number. Exports to CSV or XLSX are there if your accountant or partner wants a spreadsheet.
If you get a lot of answers, switch on "Triage submissions with AI" in Settings. Each submission gets a category, a priority, a one or two sentence summary and a suggested reply. Routing rules can email the urgent ones to up to 5 addresses. A monthly AI allowance applies per account.
Questions owners ask
How many questions should a quick feedback form have?
Four is plenty for most shops, clinics and trades. Every extra field gives someone a reason to stop. Turn on partial submissions in Settings and you can see which question people quit on.
Should I ask for a name?
Make it optional or skip it. The phone number field already covers the people who want a reply, and customers are more honest when they do not have to sign their name.
What do I do with a 1 star answer?
Reply within a day if they left a number. Apologize, ask whst happened, and fix the thing if you can. Do not ask thme to change the rating.
Can I use Google Forms instead?
Yes. Rebuild the same four fields with a linear scale, a paragraph, a multiple choice and a short answer. It works the same way, just with less control over how the form looks.
Open the template today, change "visit" to the word your customers use, and send the link to the next person you serve.
