Customer feedback form PDF: print it or collect it online

A customer feedback form PDF works if you print it, leave it by the till, and read the slips every week. Most shopkeepers do the first two and skip the third. A phone form that sends answers to your email or a spreadsheet removes the sorting and typing. Paper still wins in a few situations, covered below.

Why printed slips pile up unread

The usual story: you download a PDF, print fifty copies, and put them next to a pen. A few customers write "good" or "ok". One writes a real complaint, and you only find it three weeks later in the drawer under the receipt book.

Paper slips fail in four predictable ways:

A home baker in Pune who tucks a slip into every cake box has it worst. The customer is at a birthday party, not at the counter, and the slip goes into the bin with the cardboard.

Paper slips and a phone form side by side

Printed PDF slip Phone form
Cost Paper, ink, and your time printing Free with the right tool
Who fills it in Customers standing at your counter Anyone with the link, anywhere
Reading answers Sort and retype by hand Answers arrive in a table you can export
Replying to a complaint Only if they left contact details Ask for a phone or email and reply the same day
Spotting patterns Count tick marks yourself Charts and averages build themselves
Works without a smartphone Yes No

The last row matters. Keep it in mind before you throw the clipboard away.

When paper still wins

Paper is the right call more often than the internet admits.

A dental clinic with many older patients in the waiting room will get more answers from a clipboard than from a QR code. People are already sitting, already bored, and a pen needs no battery. Staff can type the slips into a form at the end of the day, which takes a few minutes and gives you the searchable table anyway.

Paper also wins at a stall or a pop-up where the signal is bad, and wherever you want a signed consent or comment on the spot.

A plumber in Ohio is the opposite case. Nobody carries a clipboard into a basement. He texts a link to the customer once the job is done, while the work is fresh and the customer is holding a phone.

A simple rule: if the customer is standing or sitting in your space with time to spare, offer paper as a backup. If they have already left, send a link.

You can use Google Forms, which is free with a Google account and fine for a plain set of questions. Its design and branding options are limited, though. Tally's free plan allows unlimited forms and submissions, but removing its branding, adding a custom domain, or turning on partial submissions needs the Pro pln at $29 a month.

Our team builds Qwenvo, a form builder that is completely free, and these are the things you get at $0 that most builders charge for: no badge on your form, a custom domain, unlimited teammates, partial submissions, visit and drop-off analytics, AI triage of every submission, custom design and CSS, and 180+ templates.

Here is a setup that works for a shop:

  1. Open the customer feedback template and delete anything you won't act on.
  2. Keep one rating question (a 1 to 5 scale), one open question like "What should we fix first?", and an optional phone or email field.
  3. Add a question for permission: "Can we share your comment on our page?"
  4. In Settings, turn on email notifications so every answer lands in your inbox.
  5. Copy the public link from the Share tab.

If you want the form to live at forms.yourshop.com, the Domains page walks you through adding a subdomain.

The AI triage setting sorts each answer by category, priority, and sentiment and writes a short summary with a suggested reply. You can set a rule to email the urgent ones to up to five addresses, so a furious review gets seen the same day.

Get customers to actually scan it

A form nobody opens is the same as a slip nobody fills in. Qwenvo has no built-in QR code, so paste yoru link into any free QR generator and print the code. The dental clinic can put it on the same sheet as the paper form, and the baker can print it on the card that goes in the cake box.

Three places work well:

Keep the form short. Three questions get answered, and ten get abandoned. After a few weeks, look at the drop-off analytics to see which question people quit on, then cut or reword it.

A weekly routine that gets feedback read

Collecting is half the job. Put fifteen minutes in your calendar, say Sunday evening.

  1. Open the submissions table and sort by priority.
  2. Reply to every unhappy customer who left a number.
  3. Note the one complaint that showed up more than once and fix it that week.
  4. Copy one good comment (with permission) onto your Instagram or Google Business Profile.

Export to CSV or XLSX if you like working in a spreadsheet.

Questions owners ask

Can I still get a customer feedback form PDF?

Yes. Print the form straight from your browser, or build a short sheet in Word and save it as a PDF. Qwenvo can also export a single submission as a PDF. One catch: those PDFs cannot draw some scripts, including Devanagari and Arabic, or emoji. If your customers write in Hindi, export to CSV or XLSX instead, which handle those characters.

Is a phone form really better than paper?

For reading and replying, yes, because the answers arrive sorted and searchable. For getting answers from people without a smartphone, no. Many owners use both and type the paper slips in weekly.

Do I need a website for this?

No. The form has its own public link that you can send on WhatsApp, put in your Instagram bio, or add to your Google Business Profile. If you do have a site, you can paste in an embed code.

Is Qwenvo proven?

It launched in October 2026, so there are few reviews so far. Start with your shop's real questions, and if it doesn't suit you, export your data and move.

Open the template today, trim it to three questions, and tape the QR code beside your till before the weekend.
