E invoice request form: collect GST bill details once

An e invoice requets form is a short page where a customer types their business name, GST number, billing address and bill number after they buy. You get every detail in one place, check it once, and issue the invoice once. Send the link in your WhatsApp reply or print it on the receipt, and the back-and-forth stops.

Why invoice requests turn into a mess

Picture a mobile accessories shop in Jaipur. A customer buys a ₹4,720 order of chargers and cases on Saturday. On Monday a WhatsApp message arrives: "Need GST bill." You ask for the GST number. A voice note comes back with a company name and fifteen characters read out fast. You type them in, the customer spots a wrong digit, and you reissue the invoice.

Each request like this costs several messages, and the details end up buried in a chat you have to scroll back through. A form changes the order of work. The customer fills in the details themselves, so a typo is theirs to catch before they press send, and you copy from a clean record.

What to ask for

Ask only for what prints on the invoice, plus what you need to match it to the sale. Every extra field is another reason to quit halfway.

Field Block type Why you need it
Bill or order number Short answer Matches the request to the sale
Purchase date Date Puts the invoice in the right month
Personal or business Multiple choice Decides which fields appear next
Business name Short answer Prints exactly as typed
GSTIN Short answer Ask them to paste it, not retype it
Billing address Long answer Goes on the invoice
State Dropdown Tells you which state the sale is billed to
Email for the invoice Email Where you send the PDF
Phone Phone number For quick questions
GST certificate File upload Optional, handy for first-time buyers

Selling in the US? A plumber in Ohio would swap the GSTIN for a tax ID or purchase order number and keep the rest. The same form works for a clinic that bills a company for an employee's visit.

Build the form

  1. Start from the purchase request form template, or press Ctrl J inside a new form and describe what you want, something like "customers request a GST invoice for a purchase they already made."
  2. Type / to add or remove blocks until you match the table above.
  3. Add conditional logic so the business name, GSTIN and certificate upload show only when the customer picks "Business." Someone who just wants a copy of their bill sees four fields, not ten.
  4. In the Settings drawer, turn on email notifications to you and confirmation emails to the customer. Consider prevent duplicate submissions too.
  5. Edit the thank-you page to promise only what you can keep, such as "We email your invoice within two working days."

Get the link in front of customers

The flrm only helps if people meet it at the moment they ask. Three places work for most small shops:

If you want a cleaner address, add a custom domain such as bills.yourshop.in on the Domains page. It is free on Qwenvo, and it looks more trustworthy than a long default link.

A routine for issuing invoices

Pick one time a day, even ten minutes, and work the queue in the same order.

Open the submissions table and sort the newest first. Check the GSTIN on the GST portal before you issue, because the form cannot verify it for you. Create the invoice in your billing software, email it, and mark the row done. If you connect Google Sheets, each request becomes a row, and you can add an "Invoiced" column of yiur own.

Teammates are unlimited, so your cashier can watch the queue and your accountant can see the same rows. At month end, export CSV or XLSX and send it over. If you do not want customer details sitting around forever, set data retention to delete submissions after a number of days.

Which form tool to use

Our team builds Qwenvo, a completely free form builder, so weigh this with that in mind. For this job, it gives you things many builders charge for: no badge on the form, a custom domain, unlimited teammates, partial submissions (so you see who started and gave up), visit and drop-off analytics, and 180+ templates. If you also take prepaid orders, the Payment block runs through your own Stripe account with a 0% Qwenvo fee, though Stripe is the only provider.

Google Forms is a fine choice if you want something running in five minutes. It is free with a Google account, but respondets must sign in with Google to upload a file, and design and branding are limited. Tally's free plan covers unlimited forms and submisions, logic, and uploads up to 10 MB per file. Removing its branding, custom domains, teammates and partial submissions are on Pro at $29 a month.

Three honest limits on Qwenvo. It launched in October 2026, so there are few reviews yet. It is hosted in the United States. And its per-submission PDFs cannot draw some scripts such as Devanagari, so if you print Hindi customer names, use the CSV or XLSX export instead.

Questions owners ask

Is this the same as a government e-invoice?

No. The form collects the customer's details. If your turnover means you must generate e-invoices through the government system, your accounting software does that step, and the form just hands it clean inputs. Ask your accountant whether it applies to you.

Can the form check that a GST number is real?

No, so spot check each one on the GST portal before you issue the invoice. Asking customers to paste the number instead of retyping it removes most typos.

Can customers upload their GST certificate?

Yes, add a File upload block and mark it optional. Qwenvo accepts files up to 1 GB each, far more than a certificate needs.

Do I need this if I only get a few requests a month?

A few requests are enough to justify it, because each one costs you the same chain of messages. The form takes minutes to build and then answers the first message for you.

Next step: open the template today, cut it down to the fields in the table, and paste the link into your next "need GST bill" reply.
