A Word customer feedback form template works if you hand it out on paper and read every sheet yourself. It stops working the moment you want to know how many customers rated you four or five, or which complaint keeps coming back. Copy the same questions into a free online form and the totals build themselves.
Where a Word template works and where it stops
A printed Word form is fine when the customer is standing in front of you. A dental clinic can leave sheets and a pen at the front desk, and a shop owner can hand one over with the bill. Nothing to log into, nothing to explain.
The trouble starts the next morning. Handwriting needs retyping. Forms sent back by email arrive as ten separate attachments. A home baker in Pune who sends the file on WhatsApp after each delivery gets back photos of half-filled pages, and can't say whether customers liked the new cake sizes without reading every one.
| What you want to know | Word file | Online form |
|---|---|---|
| How many people gave 4 or 5 | Count by hand | Chart and average |
| Which comments mention delays | Read every sheet | One table of answers |
| Who needs a call back | Spot it yourself | Email to you when a form comes in |
| Moving answers to a spreadsheet | Retype | Export as CSV or XLSX |
| Collecting from WhatsApp customers | Send a file, hope they print it | Send one link |
An online form removes the retyping because each answer lands in a table the moment someone submits.
Use the free feedback form template
Move your Word questions into an online form
Qwenvo is a completely free form builder, and our team builds it, so I'll use it for the steps. The same approach works in other tools.
- Open your Word file and cut it down to the questions you wipl actually use. Seven is plenty.
- Create a form and import the file. Qwenvo reads Word (.docx), and also PDF, CSV, Markdown, plain text and screenshots. The import guide has the details. If you would rather start clean, use the free customer feedback form template, or describe the form in a sentence and let the AI draft it.
- Check each question's type after importing. A row of tick boxes in Word might need to become Multiple choice, and a "rate us" line should become a Rating block.
- Type
/to add or swap blocks. The editor works like a document, so it feels close to Word. - Open the form's Settings. Turn on email notifications so each answer reaches you, and set a thank-you page.
- Open the link on your own phone and fill it in as a customer would. Most of your customers will do the same.
Which questions are worth keeping
Paper forms tend to grow. Online, every extra question costs you completed forms, so keep the ones you will act on.
| Line in your Word template | Block to use | What you get |
|---|---|---|
| Rate our service | Rating | An average and a chart |
| Which service did you use? | Dropdown | Answers you can compare by service |
| What could we do better? | Long answer | Every comment in one table |
| Would you recommend us? | Linear scale | NPS and averages in Insights |
| Name and phone (optional) | Short answer, Phone number | Someone to call back |
Add one piece of logic. If the rating is 1 or 2, show a "What went wrong?" box and ask whether you may call. A plumber in Ohio only needs that follow-up from unhappy customers, and happy ones never see it. In Qwenvo that is the "Add conditional logic" option in a question's menu.
Reading and sharing the answers
Answers go itno a Submissions table. You can export them as CSV, JSON or XLSX, and Insights shows charts and averages. If people open the form and leave halfway, partial submissions appear as incomplete rows, and drop-off analytics show where they quit. If many open it and few finish, shorten it.
AI triage helps when you get more replies than you can read. Each submission gets a category, a priority, a sentiment, a short summary and a suggested reply. Routing rules can email an urgent complaint to up to five addresses, say you and your manager. A monthly AI allowance applies per account.
For sharing, the public link goes anywhere: a WhatsApp message after a delivery or a job, your Instagram bio, a text after an appointment. Qwenvo has no built-in QR code, so paste the link into any QR generator and print the result for your counter. Our post on a feedback form with a QR code covers that. You can also put the form on your own subdomain, such as forms.yourbusiness.com, by adding a CNAME and a TXT record on the Domains page.
Most form builders charge for several of these. Tally's Pro plan is $29 a month, and that is where branding removal, custom domains, teammates, partial submissions, custom CSS and visit and drop-off analytics sit. On Qwenvo they are free, along with AI triage, custom design, unlimited teammates and 180+ templates. There is no badge once you switch the branding toggle off.
Other options, and where Qwenvo falls short
Google Forms is free with a Google account and fine for a short form. Its design and branding options are limited, and customers must sign in with Google to upload files. If everything you do already runs through Google, it may be all you need.
Qwenvo has limits too. It launched in October 2026, so there are few reviews yet. Per-submission PDFs can't draw some scripts such as Devanagari or Arabic, or emoji. If your customers answer in Hindi, export CSV or XLSX instead, which handle them fine. The form itself can run in 45+ languages.

Questions owners ask
Can I import my Word template directly?
Yes. Qwenvo accepts .docx files. Check the question types afterward, since a Word file only has text and the builder needs to know which lines are ratings, choices or comment boxes.
Is it really free?
The price is $0, with no paid plans, no card and no trial. A faair use policy applies if an account reaches around 50,000 submissions or 50,000 emails a month, or 100 GB of uploads. A local business will not come close.
Can I still keep a printed copy?
Keep your Word file for the front desk. You can type the paper answers into the same online form latdr, so everything ends up in one table. Our customer feedback form template post has wording ideas if you want to refresh the questions.
Do I need a website to use this?
No. The form has its own link, which you can send on WhatsApp or put in an Instagram bio. If you do have a site, the Share tab gives embed code you can paste into any builder that accepts HTML.
