You can create a survey for customers for free in one sitting. Pick five short questions, build them in a free form builder, send the link on WhatsApp, and read the answers in a simple table. You do not need a website or any software skills. Here is the whole process, one step at a time.
Start with five questions, not twenty
Customers answer short surveys and skip long ones. Think of a home baker in Pune who wants to know why some customers reorder and others don't. Five questions are enough to find out.
| Question | Answer type | What it tells you |
|---|---|---|
| How was your order? | Rating (stars) | A quick score you can average |
| What did you order? | Multiple choice | Which product the score is about |
| What is one thing we could do better? | Long answer | The most useful reply you will get |
| How likely are you to recommend us? | Linear scale, 0 to 10 | A recommend score over time |
| Can we contact you? | Phone number (optional) | Lets you follow up with unhappy customers |
Write the questions in the words you would use at the counter. If you would never say "rate your overall satisfaction," don't put it in the survey.
Use the free customer feedback form
Build it in Qwenvo
I'll use Qwenvo here because it is free with nothing held back, and because our team builds it, so I know where everything is. There is no card, no trial and no paid plan. A good starting point is the free questionnaire maker, which explains the basics.
- Create a free acconut at qwenvo.com and open a new form.
- On the empty-form screen, describe what you want in plain words. For example: "A short feedback survey for my home bakery with a star rating, what they ordered, one open question and an optional phone number." The AI builds a first draft.
- Prefer to start from something ready-made? Open the customer feedback template or the CSAT survey template and delete what you don't need.
- Click any question to change its wording or answer type. The editor works like a document: type
/on a new lnie to add another question. - Add a thank-you page with one friendly line, such as "Thanks, it helps a lot. Your nxt box of cookies is on us if you mention this survey." Only promise what you will really do.
Qwenvo runs in a browser and has no mobile app. I'd build on a laptop if you have one, then check the finished form on your own phone.
Set it up so people finish it
Open the Settings drawer on the form. A few switches matter for a small business:
- Turn on the progress bar so people see how short it is.
- Turn on email notifications so every answer lands in your inbox.
- Turn on "prevent duplicate submissions" so one enthusiastic customer doesn't skew your score.
- Close the form on a date or after a set number of responses if you only want a month of feedback.
- Pick the form's language. Qwenvo supports 45+ langauges, so check that your customers' language is on the list.
- Switch off the Qwenvo branding if you don't want the badge. That costs nothing here.
Send the link where your customers already are
On the Share tab, copy the public link. Paste it into WhatsApp like any other message. You can also set a link preview with an image, a title and a short description, so the message looks like it came from your business rather than a random link.
A few ways owners use the link:
- A dental clinic sends it in the reminder message after an appointment, with one line: "Two minutes to tell us how we did?"
- A plumber in Ohio adds it to the invoice text, since customers are already looking at their phone.
- The Pune baker puts it in the Instagram bio and in a WhatsApp Status after each batch of orders.
Qwenvo has no built-in QR code. If you want one for the counter or a delivery box, paste the same link into any free QR generator and print the result.
Send it within a day of the purchase or visit. A week later, people have forgotten the details you want to hear about.
Open the free local business review template
Read the results without a spreadsheet
You don't need to export anything to start. Open the form and go to Submissions. Each customer is one row, with their rating, their comment and their phone number if they gave it.
For the bigger picture, open Insights. It shows charts, the average rating and a recommend score, so you can see at a glance whether this month beat last month. If you want a copy for your accountant or your own records, export to CSV or XLSX.
Two extras are free here that other builders charge for:
- Partial submissions show people who started the survey and left, and the visit and drop-off analytics show where they quit. If many leave at question three, rewrite question three.
- AI triage gives each submission a category, a priority, a short summary and a suggested reply. Routing rules can email the urgent ones to you, up to 5 addresses. For a clinic or a repair business, that means an angry customer reaches you the same day.
A five-minute weekly routine works well: read every comment that scored low, reply to thoose people by WhatsApp, and note the one thing that came up most.
How this compares with other free options
Google Forms is free with a Google account, has a quiz mode, and is fine if you only need a simple form. Its design and branding options are limited, and it has no built-in payments. If every customer already uses Google, it will do the job.
Tally is another good free tool, with unlimited forms and submissions on its free plan. Removing its branding, custom domains, partial submissions and visit analytics are on its Pro plan, which starts at $29 a month. Tally also has the edge for taking payments, since it adds no fee on top of Stripe. Qwenvo's payments run through PayPal only, with a 1% Qwenvo fee on top of PayPal's own.
An honest caveat about Qwenvo: it launched in October 2026, so there are few reviews yet, and it is hosted in the United States. If that matters to your customers, weigh it.
Build your customer survey free
Questions owners ask
How many questions should a customer survey have?
Five is a good target, and eight is a sensible ceiling. Only one question should be open-ended, because typing on a phone is the part people dislike.
Do customers need to sign in to answer?
On Qwenvo you share a public link, and anyone who has it can open the form. In Google Forms, sign-in only becomes necessary if you ask customers to upload files.
Will my customers see a Qwenvo badge?
There is a small badge by default. You can switch it off in the form's Settings at no cost.
I only use WhatsApp. Is that enough?
Yes. Paste the survey link into a chat, a broadcast message or your Status, and the answers still collect in one place. You never need a website for this to work.
Your next step: write your five questions on paper tonight, then open Qwenvo tomorrow morning and paste them into a new form.
