An order form is a document where a customer tells you exactly what they want to buy, in what quantity, and where and when to deliver it. It replaces the back-and-forth of WhatsApp messages with one clear request. You get everything needed to fill the order, and the customer gets proof of what they asked for.

Order form, quote, and invoice: what's the difference
New sellers often mix these up because all three have items and prices on them. The difference is who sends it and when.
| Document | Who sends it | When | What it does |
|---|---|---|---|
| Quote | You | Before the customer commits | Offers a price for a job or a set of items |
| Order form | The customer | When they decide to buy | Records what they want and how to reach them |
| Invoice | You | After the order, or at delivery | Asks for payment and records what was charged |
A quote is your offer. An order form is their yes. An invoice is your bill. If you sell standard items at fixed prices, like cakes, tiffin boxes or candles, you can often skip the quote and start at the order form. If every job is different, like a bathroom repair or a custom cabinet, send a quote first and use the order form to confirm the choice.
Fields every order form needs
Keep it short. Each extra question costs you a few customers who give up halfway on their phone. These fields cover most small businesses:
| Field | Why you need it | Tip |
|---|---|---|
| Name | So you know who to address | One field is enough |
| Phone number | Your fallback when something is unclear | Make it required |
| For a confirmation copy | Optional if you work mostly on WhatsApp | |
| Item and quantity | The core of the order | Use multiple choice or a dropdown so nobody has to type your menu |
| Variations | Size, flavor, color, message on the cake | Show only the ones that apply to the chosen item |
| Delivery or pickup | Changes the next questions | Ask this before asking for an address |
| Address | Only for delivery | Hide it when they pick pickup |
| Date and time | When they need it | Use a date picker, not free text |
| Notes | Allergies, gate codes, gift wording | One long answer box |
| Total price | Removes surprises | Calculate it automatically if you can |
| Payment method | Cash on delivery, UPI, card, or pay online | List only what you actually accept |
Add a line near the top with your cutoff times. "Orders for the weekend close Thursday 6 PM" saves more arguments than any field does.
What it looks like in practice
A home baker in Pune might offer three cake sizes and a handful of flavors. The form asks for size, flavor, the message on the cake, picukp or delivery, and the date. If a 1 kg chocolate cake is ₹1,200 and delivery is ₹100, the total can show before the customer submits. Nobody has to ask "how much for delivery?" in a DM again.
A plumber in Ohio who sells fixed-price jobs, such as a water heater swap, needs different fields: job type, tank size, street address, a preferred date, and a photo of the current setup. A file upload field handles the photo. If the job is not a standard one, point the customer to a quote request form instead.
Building one online
You can build an order form on paper, in a spreadsheet, or in a form builder. A form builder is the easiest for customers on phones, and it collects every order in one list.
Qwenvo is a completely free form builder, and our team builds it. You type / to add a block, so the editor feels like writing a document. For an order form you will mostly use multiple choice, number, date, phone number and long answer blocks. Calculated fieelds add up prices, and conditional logic hides the address question when someone picks pickup.
You can also describe form to the AI builder and edit what it gives you, or start from one of the 180+ templates.
Here is what you get free that many form builders put behind a paid plan:
- Removing the Qwenvo badge
- Your own domain, such as
orders.yourbakery.com - Teammates, so a staff member can see orders too
- Partial submissions, so you see people who started and stopped
- Visit and drop-off analytics
- Custom design and CSS
- AI triage that labels each submission and suggests a reply
Tally is a good alternative. Its free plan covers unlimited forms and submissions, logic, calculations, payments and file uploads up to 10 MB per file. Branding removal, custom domains, teammates and partial submissions are on its Pro plan at $29 a month. Google Forms is free with a Google account and fine for simple orders, but it has no built-in payments and little control over design.
Sharing it and getting paid
Once the form is ready, copy the public linnk. Paste it into your WhatsApp Business replies, your Instagram bio, and your Google Business Profile. Customers tap, fill, and submit. You get an email for each order, and the customer can get a confirmation copy.
Qwenvo has no built-in QR code. If you want one for a shop counter, paste the link into any QR generator.
For online payment, Qwenvo's Payment block works through PayPal checkout only. Qwenvo kees a 1% platform fee on top of PayPal's own fee. If payments are the main reason you want a form, Tally is cheaper because it adds no fee on top of Stripe. Many small sellers skip online payment at first and take UPI, cash or a bank transfer at delivery. That works fine with a form that records the order.
A weekly routine that keeps orders tidy
Setting up the form takes an afternoon. Keeping it useful takes about ten minutes a week.
- Check the new orders each morning and reply to anything marked unclear.
- Close the form on a date or after a set number of responses when you reach capacity.
- Export the ordders to a spreadsheet for your shopping and prep list.
- Look at the drop-off numbers once a month. If many pople leave at the address question, shorten it.

Questions owners ask
Is an order form legally binding?
It depends on where you live and what you sell, so I can't gkve you a rule. It does record what the customer asked for and when, which settles most small disputes. If you sell high-value or custom work, ask a local professional how to word your terms.
Do I need an invoice if I have an order form?
Not always. For small cash sales, the order confirmation is often enough. If a customer is a business, or you are required to issue invoices where you live, send one after the order.
Can I take orders on WhatsApp instead?
You can, and many sellers do. The problem is that details get scattered across a long chat. A form asks for everything once, so you can still reply on WhatsApp, only with the information already in hand.
How long should an order form be?
As short as your product allows. A cake order might need eight fields. A repair job might need ten. If you find yourself writing twenty, split it into pages or ask for the rest after the order is placed.
Your next step
Open the order form template, delete the fields you don't need, add your items and prices, and send the link to your next three customers. Ask them if anything felt confusing, thrn fix that one thing.
