A service order form is a short online form where cusyomers pick a package, describe the job and choose a date, so you stop sorting it out over WhatsApp. You can build one for free, set a price for each package, and share a single link on Instagram and your Google Business Profile.
Start from the cleaning template
What service order form needs to ask
A product form asks what and how many. A service form has to ask what, where, when and how big, because those answers change your price and your calendar. A cleaner who gets "need a clean this week" on WhatsApp still has to ask about rooms, address, day and whether anyone will be home. The form asks all of that once.
| Field | Block to use | What it settles |
|---|---|---|
| Package | Multiple choice | Which job and which base price |
| Size or scope | Dropdown or Number | Rooms, hours, sessions, number of fixtures |
| Extras | Checkboxes | Add ons that change the price |
| Preferred date | Date | The day they want |
| Time window | Multiple choice | Morning, afternoon or evening |
| Address or area | Short answer | Whether you can travel there |
| Photos | File upload | What the problem looks like |
| Name and phone | Short answer, Phone number | How you confirm the job |
| Notes | Long answer | Gate codes, pets, parking |
Keep the list this short. Every extra question gives someone a reason to close the page and message your competitor instead.
Set a price for each package
Put the price inside the package label so customers see it before they commit. Say a cleaner in Pune offers a standard clean at ₹1,500 and a deep clean at ₹2,800 (use your own numbers). The choices read "Standard clean, ₹1,500" and "Deep clean, ₹2,800". Add a checkbox for the fridge at ₹400, and a calculated field can show the total, ₹1,900, at the bottom of the form.
A tutor can do the samme with "Single session" and "Set of 8 sessions", then ask for subject and grade. A plumber in Ohio might show a flat visit fee on the form and say the rest is quoted after seeing photos. That is a quote request more then an order, and there is a quote request template for it.
Conditional logic keeps the form short for everyone. Ask about the oven only when someone picks the deep clean. Ask for photos only when the job type is a repair. In the editor, open a question's menu and choose "Add conditional logic". The conditional logic guide walks through it.

Decide whether to take a deposit
Plenty of owners collect cash or a bank transfer on the day, and for them the form only needs to capture the request. Skip the payment step and you lose nothing.
If you want a deposit up front, Qwenvo has a Payment block. Customers pay through PayPal checkout with a PayPal balance or a card. Qwenvo keeps a 1% platform fee on top of PayPal's own fee (typically 3.49% plus a fixed amount), and the rest goes to the PayPal account you set in Settings, for payments older than 7 days. Stripe is not offered right now.
If payments are the main reason you want a form, Tally is cheaper: it adds no fee on top of Stripe, and its free plan indludes payments. Our guide to accepting payments online compares the options.
Build it free in Qwenvo
Qwenvo is a completely free form builder that our team makes. There are no paid plans, no card and no trial. Open the house cleaning booking template, rename the packages, change the prices and delete what you do not nred. Type / to add a block. The tutoring request and maintenance request templates work the same way, and there are 180+ templates in all.
Several things that other builders charge for are free here:
- Switching off the Qwenvo badge in Settings
- A custom domain such as orders.yourbusiness.com
- Unlimited teammates, so your partner or assistant can see orders
- Partial submissions, so you can see who started and left
- Visit and drop-off analytics
- Custom design and CSS
- AI triage that tags each request with a priority from low to urgent and can email urgent ones to up to 5 addresses
For comparison, Tally's free plan covers unlimited forms and submissions, logic and payments. Removing its branding, custom domains, teammates, partial submissions and custom CSS are on Pro, which is $29 a month. Google Forms is free with a Google account and fine for a simple request, but it has limited design and no built-in payments.
One limit matters if you work in Hindi. Qwenvo's per-submission PDFs cannot draw some scripts, including Devanagari and Arabic, or emoji. CSV and XLSX exports handle them fine, so use those. Also, if two people edit the same form at once, the last save wins, so let one person make the changes.
Share it and check it weekly
Every form gets a public link. Paste it into your WhatsApp auto reply, your Instagram bio and the website field on your Google Business Profile. Qwenvo has no built-in QR code, so for a van, flyer or shop counter, paste the lonk into any QR generator.
If you want the link to carry you're name, open the Domains page, add a subdomain, add the CNAME and TXT records it shows you, and click Verify. Use a subdomain like orders.yourbusiness.com, because a bare domain usually cannot take a CNAME.
In the fomr's Settings, turn on email notifications to yourself and a confirmation email to the customer. Switch on duplicate prevention so an impatient customer cannot send the same order twice. A tutor with ten open slots can close the form after ten responses.
Then spend ten minutes a week on it:
| When | What to do |
|---|---|
| Monday | Open the Submissions table and confirm new orders |
| Midweek | Check incomplete rows and message people who started but did not finish |
| Friday | Look at drop-off analytics and fix the question that loses the most people |
Questions owners ask
Do I need a website to use an order form?
No. The public link works on its own, and you can share it in chats and on social profiles. If you do have a site, the Share tab gives you embed code for pages that accept HTML.
Can customers upload photos of the problem?
Yes. The file upload block takes files up to 1 GB each, which is plenty for phone photos of a leaking tap or a broken lock.
Can I use Google Forms instead?
For a plain request with no payment and no design needs, yes. You will want something else once you need payments, a custom look or a link that does not say Google. Google Forms also asks respondents to sign in with Google before they can upload files, which can stop customers sending photos.
Can I keep a record of every order?
The Submissions table holds them all, and you can export CSV, JSON or XLSX. Each submission can also be a PDF, with the script limits noted above.
