Camp registration form PDF: keep paper, collect online

You don't have to pick between a camp registration form PDF and an online form. Put the sign-up online so every answer lands in one table, then print what you need: a PDF of each camper's submission for your binder, and your original PDF for the few parents who still want paper.

Why paper alone gets slow

Picture a summer cricket camp in Pune. Parents photograph the filled form and send it on WhatsApp. Someone retypes the phone numbers into a spreadsheet. Two forms have the allergy line blank, and you only notice on day one.

Every step after the parent hits send is your time. An online fofm does the typing for you, and it can refuse to submit until the emergency contact is filled in. A paper form cannot.

What goes online and what stays on paper

Almost everything can move online. The only things that really need paper are a handful of walk-in registrations and anything your venue insists on signed in ink.

Field Type Why it is there
Camper name and date of birth Short answer, Date Roster and age groups
Parent WhatsApp number Phone number Where you will actually reach them
Emergency contact Short answer, Phone number Needed on day one
Any allergies or medical needs? Multiple choice (yes/no) Opens the next field only if yes
Details of allergies or medical needs Long answer Shown only when the answer above is yes
Who may pick up Long answer Names and relationships
Photo permission Multiple choice Social media posts
Parent signature Signature Replaces the signature line
Camp fee Payment Optional, see the payment note below

Ask only what you need to run the camp. Medical details are sensitive, so collect the minimum and delete it when camp ends.

Build it from the PDF you already have

You do not need to start from a blank page. Qwenvo (our team builds it, and it is completely free) can import a PDF, a Word file, a screenshot or plain tet and turn it into a form.

  1. Open the empty-form screen and import your existing camp form. The import guide lists the supported file types.
  2. Clean up the result. Type / to add or change a block, and swap long text boxes for multiple choice where you can.
  3. Add the allergy logic: show the details box only when the parent answers yes.
  4. Add a Signature block at the end, and a Payment block if you want to collect the fee.
  5. Open Settings and turn on a confirmation email to the parent, an email notification to you, and progress bar if the form runs over several pages.
  6. Set the form to close after a number of responses equal to your camp capacity, so you never overbook.
  7. Share the link in your WhatsApp group, your Instagram bio and on the poster.

Import your PDF form free

A note on payments. Qwenvo takes them through PayPal only, and it keeps a 1% fee on top of PayPal's own fee. If most parents pay by UPI, cash or bank transfer, skip the Payment block and write the instructions in a Text block above the submit button. Tally is cheaper on card payments because it adds no fee on top of Stripe.

Tell parents which way to sign up

Parents follow whatever you tell them, so say it plainly in one message. Something like this works in a WhatsApp group:

"Registration for summer camp is open. Please fill in this form: [link]. If you'd rater use paper, reply here and I'll send you the PDF."

On the printed flyer, put the link in large type next to a QR code. Qwenvo has no built-in QR coe, so paste your form link into any free QR generator and print the result. Parents at the school gate can scan it on the spot, and the ones who prefer paper take the sheet home.

Keep the printable copy

Once parents have submitted, you can still end up with paper.

One limit matters here. The per-submission PDF cannot draw some scripts, including Devanagari and Arabic, or emoji. If parents write names in Hindi or Marathi, print your roster from the XLSX or CSV export instead.

Other tools worth a look

Google Forms is free with a Google account and fine for a simple sign-up list. It has no built-in payments, and parents must sign in with Google to upload files, which gets awkward if you ask for a vaccination record. Design and branding options are limited.

Tally has a generous free plan with unlimited forms and submissions, logic, signatures, payments and file uploads up to 10 MB each. Its Pro plan costs $29 a month and is where branding removal, custom domains, teammates, partial submissions and visit and drop-off analytics live.

On Qwenvo those are free: no badge (switch the branding off in Settings), a custom domain such as forms.yourcamp.com, unlimited teammates, partial submissions, visit and drop-off analytics, custom dezign and CSS, and 180+ templates. If your camp runs on a volunteer team, the teammate access alone can matter.

Two honest cautions. Qwenvo launched in October 2026, so there are few reviews yet, and it is hosted in the United States, while Tally is hosted in Europe. It also has no HIPAA offering, so if a health provider is involved, check what they require before collecting medical forms anywhere.

A simple weekly routine

When What you do
Two weeks out Share the link, check the incomplete rows
One week out Export the XLSX roster, call parents with missing details
Day before Print the submission PDFs for the binder, plus the emergency contact sheet
After camp Delete the data, or set auto-delete in Settings after a set number of days

Questions owners ask

Can I just email the PDF and have parents send it back?

You can, and it works for a very small camp. The cost is that you retype or chase every answer yourself, and you cannot see who has not replied. An online form fixes both and still lets you print.

Can parents sign the form on their phone?

Yes, there is a Signature block. Whether an online signature is enough for your insurer or venue is something to confirm with them first. If they want ink, keep the PDF handy for that one step.

How do I stop the camp from overbooking?

In the form Settings, close the form after a set number of responses. For a waitlist, you can point parents to a second form from the thank-you page.

Is it really free, with no catch?

The price is $0, with no card and no trial. There is a fair use policy for very large accounts, around 50,000 submissions a month, which a camp will not come near.

This week, import your current camp PDF, add the allergy logic and send the link to one parent as a test before you share it with everyone.
