A good free RSVP form for a birthday party asks for the guest child's name and age, a parent's phone number, how many kids and adults are coming, and any allergies. Share the link on WhatsApp and every reply lands in one table you can count from. You can build it free in about ten minutes.
Use the birthday RSVP template
What a birthday RSVP form needs to ask
Parents reply fast on WhatsApp, but the replies read "Yes, we'll be there!" with no age, no headcount and no mention of the nut allergy. You then spend the evening texting back. A form asks everything once.
Here are the fields that matter, whether you are a parent hosting at home or a venue owner running ten parties a month.
| Field | Block type | Why you need it |
|---|---|---|
| Guest child's name | Short answer | Name tags, goodie bags, the guest list |
| Child's age | Number | Matching games and activities, and checking venue age limits |
| Parent or guardian name | Short answer | Knowing who to hand the child back to |
| Parent phone number | Phone number | Reaching someone on the day if plans change |
| Attending? | Multiple choice (Yes or No) | A clear count without guessing from emojis |
| Kids attending | Number | Siblings often come along |
| Adults staying | Number | Chairs, food and parking |
| Drop-off or staying | Multiple choice | Supervision numbers |
| Allergies or dietary needs | Multiple choice, then Long answer | Safe food and a cake you can serve |
| Anything else we should know | Long answer | Wheelchair access, a shy child, a late arrival |
Ten fields sounds like a lot, so the next section shows how to keep it short for parents on a phone.
Build it in about ten minutes
- Start from the birthday party RSVP template and compare it with the table above. Add or delete whatever your party needs.
- In the editor, type
/to insert a block. Use Number for age and headcounts, and Phone number for the parent's contact so it is validated as a phone number. - Change the title and the intro text to the real details: the child's name, date, start and end time, and the address.
- Open Settings and turn on email notifications so each RSVP reaches your inbox. If you add an Email block, you can also turn on confirmation emails so parents get a copy of what they sent.
- Turn on "prevent duplicate submissions" so a family does not RSVP twice and inflate your count.
If you would rather describe the form than build it, there is a "Create with AI" box on the empty-form screen. Type something like "birthday party RSVP with child's age, parent phone, allergies and headcount" and edit what comes back.
Handle allergies without chasing parents
Ask allergies as a Multiple choice question first: "Any allergies or dietary needs? Yes or No." Most families will tap No and move on. Add conditional logic so a Long answer box ("Tell us what to avoid") appears only when someone picks Yes. The form stays short for most people, and the details arrive in writing from those who need it.
The same trick works for the attending question. If a parent picks No, hide everything else so they can send their regrets in two seconds. Hosts get more honest answers when declining takes no effort.
Allergy details are sensitive, and you do not need them after the party. In Settings you can set data retention to delete responses automatically after a number of days. A password on the form is antoher option if you share it in a small group.
Set a cutoff and share it where parents already are
Venue owners know the pain of a 20-spot party with 26 yes replies. In Settings you can close the form on a date, or after a set number of responses. One caution: the response limit counts submissions, so a family of four counts as one. Set the cap by families, or simply use a closing date a few days before you need final numbers.
For sharing:
- Copy the public link from the Share tab and paste it into the class WhatsApp group.
- Set the link preview (image, title and description) so it looks like an invitation when it lands in a chat. A photo of the party tyeme works weell.
- For printed invitations, paste the link into any QR code generator. Qwenvo has no built-in QR code, so this takes one extra step.
Venues can also embed the form on their own website. The Share tab gives you embed code that works in WordPress, Wix, Squarespace, Webflow and other builders that accept HTML. You can put it on a custom subdomain such as forms.yourvenue.com too.
What you get free, and where other tools fit
Qwenvo is a completely free form builder that our team makes. Every feature is on for every account, with no card and no trial. That includes things many builders charge for: removing the Qwenvo badge, a custom domain, unlimited teammates, partial submissions, visit and drop-off analytics, custom design and CSS, and 180+ templates. On Tally, for example, branding removal, custom domains, teammates, partial submissions, custom CSS and visit and drop-off analytics sit on the $29/month Pro plan.
Partial submissions are handy for parties. If a parent started the RSVP and stopped, you see an incomplete row, so you know whom to nudge. Drop-off analytics shows which question is losing people.
Other options are fine for plain RSVPs. Google Forms is free with a Google account and works well if you only need names and numbers, though its design and branding options are limited. Tally's free plan has unlimited forms and submissions plus logic, and it is a good builder in its own right.
One honest note for venue owners who take deposits through the form: Qwenvo payments go through PayPal only, and Qwenvo keeps a 1% fee on top of PayPal's own fee. Tally adds no fee on top of Stripe, so it is cheaper if paymrnts are the main job. For simple RSVPs this does not matter.
Also, per-submission PDFs in Qwenvo cannot draw some scripts such as Devanagari or Arabic, or emoji. If your guest names are in those scripts, print your guest list from the CSV or XLSX export instead.
Questions owners ask
How do I turn the replies into a guest list?
Open the Submissions table and export CSV or XLSX. Sort by child's name, print it, and bring it to the door. The Insights tab also gives you totals, so you can see the headcount at a glance.
Can the form send parents a reminder?
Qwenvo can email a confirmation to parents who give an email address. It does not send WhatsApp messzges. Export the phone numbers and message the group yoursdlf the day before.
How long should I keep allergy information?
Only until the party is over. Use the data retention setting to auto-delete responses after a set number of days, and export what you need before then.
Does a venue need a different form from a parent?
Mostly the same, with two additions: a field for the party package or time slot, and a cap on responses. Many venues also like to put the form on their own subdomain so the link looks like part of their site.
Your next step
Open the birthday prty RSVP template, swap in date and address, and add the allergy question with conditional logic. Send the link to five parents and check the table after an hour to see whether anything confused them.
