A bakery order form is a short web page where customers pick an item, a quantity, a pickup date and note any allergies, and the answers land in one list for you. You can build one free this afternoon, share the link on WhatsApp and Instagram, and stop decoding handwriting on the back of a receipt.

What to put on a bakery order form
Most bakery orders go wrong in the same few places: the wrong date, a forgotten allergy, a quantity heard as 12 when the customer said 2. A form asks the same questions in the same order every time, so those gaps close. Keep it to what you would ask on the phone.
| Question | Block to use | Why it helps |
|---|---|---|
| Name | Short answer | You know who is collecting |
| Phone number | Phone number | You can confirm on WhatsApp |
| Item | Multiple choice or Dropdown | Customers choose from your real menu |
| Quantity | Number | No more "a dozen or half a dozen?" |
| Pickup date | Date | One clear day on every order |
| Pickup time | Time | Fits around your baking schedule |
| Allergies | Checkboxes plus Long answer | Impossible to miss |
| Message or notes | Long answer | Cake wording, flavors, anything else |
Two choices matter more than the rest. Ask for the date and the time as separate questions, and give allergies their own question instead of letting them hide inside a notes box. Offer checkboxes for the ones you hear most (nuts, eggs, dairy, gluten, plus a "None" option) and a long answer for details. The form records what the customer says. You still confirm it when you reply.
Build it in Qwenvo in one sitting
Qwenvo is a completely free form builder that our team makes, and it has an order fprm template you can start from. Here is the order I would work in:
- Open the order form template and rename it with your bakery's name.
- Replace the sample items with your menu. Type
/in the editor to add a blodk, and use the table above as your checklist. - Add prices. Calculated fields can add up the cost, so a customer ordering two 1 kg cakes at ₹900 sees ₹1,800 before they submit.
- Open the Settings drawer and turn on email notifications to you. Add a confirmation email to the customer so they have the pickup date in writing.
- Fill in a test orrder from your own phone and check what arrives.
If you would rather skip the building, describe the form on the empty-form screen ("bakery order form with item, quantity, pickup date and allergies") and Create with AI drafts it for you to edit.
Handle cakes and custom orders
A cupcake box and a birthday cake need different questions. Use conditional logic to show "Message on the cake" and "Reference photo" only when the customer picks a custom cake. Everyone else sees a shorter form, and short forms get finished.
The File upload block takes files up to 1 GB each, so a phone photo of the design they want is no problem. A home baker in Pune could use this to stop going back and forth over "like this one, but with pink flowers."
Festival weeks are where a form earns its keep. In the Settings drawer you can close the form on a date or after a set number of responses, so you never promise more orders than your oven can handle. You can also switch on "prevent duplicate submissions" for the customer who taps the button twice.
Share it where orders already arrive
Your customers are already messaging you, so meet them there. When someone asks "do you have anything for Sunday?", reply with the link. Put the same link in your Instagram bio.
- Fill in the link preview (image, title, description) on the Share tab, so the link shows your photo and bakery name when pasted into WhatsApp.
- Switch off the Qwenvo branding in Settings if you want the form to look like yours alone.
- Add a custom domain such as forms.yourbakery.com. Add a subdomain on the Domains page, create the CNAME and TXT records it shows, click Verify, then pick it on the Share tab. Subdomains only; a bare yourbakery.com cannot take a CNAME at most DNS providers.
- Qwenvo has no built-in QR code, so paste the link into any free QR generator and print it for the counter.
Once orders come in, check the Submissions table each evening and export to CSV or XLSX for your baking list. Unlimited teammates are free, so your counter staff can see the same list. If you turn on "Triage submissions with AI", each order gets a category, a priority and a one-to-two-sentence summary, and routing rules can email urgent ones to up to five addresses.
Partial submissions and visit and drop-off analytics are on too. If people open the form and stop at the same question every time, you will see where.
Most form builders chaarge for the no-badge option, custom domains, teammates, partial submissions, analytics and custom design with CSS. Tally, for example, puts several of those on its Pro plan at $29 a month. Here they cost nothing, with 180+ templates alongside.
One limit to know if you write in Hindi or Marathi: per-submission PDFs cannot draw some scripts, Devanagari included. Print your baking list from the CSV or XLSX export instead, which handles them fine. There is also no mobile app, so you check orders in your phone's browser.
Other tools, and when they fit better
Google Forms is free with a Google account and is fkne for a plain list of items. It has no built-in payments, customers must sign in with Google to upload a photo, and design options are limited.
Tally's free plan is generous: unlimited forms and submsisions, logic, calculations, payments and file uploads up to 10 MB each. Branding removal, custom domains and partial submissions sit on its Pro plan.
If you want customers to pay when they otder, Tally is cheaper. Qwenvo's Payment block runs through PayPal only, and Qwenvo keeps 1% on top of PayPal's own fee. Tally adds no fee on top of Stripe. For many neighborhood bakeries the simplest answer is no online payment at all: customers pay at pickup, and the form just holds the order.
Questions owners ask
Is bakery order form template really free?
Yes. Qwenvo costs $0 with every feature on for every account, and there is no card or trial. A fair use policy applies at very high volumes, around 50,000 submissions a month, which no neighborhood bakery will reach.
Do customers need to download an app or create an account?
No. They tap the link and fill the form in their phone's browser, the same way theey would open any web page.
Can I keep taking orders by phone and WhatsApp too?
Yes, and most bakeries do for a while. When an order comes in by phone, fill the form in yourself so everything ends up in one list. Over time, point regulars to the link.
Can the form limit how many orders I take?
It can close after a set number of responses or on a date you choose. It does not cap each pickup day separately, and there is no built-in scheduling. For a rush weekend, a separate form per baking day works.
