A good wedding caie order form asks for the wedding date, venue, serving count, flavors, tasting slot and deposit terms before you name a price. Keep a printed copy for bridal expos and your shop counter, and an online version for everyone who finds you on Instagram or WhatsApp. Both should ask the same questions in the same order.

What the form must capture
A wedding cake is often the most expensive order on your calendar, and a price given without the right details is a guess. A venue with no lift, an outdoor reception in summer, or a guest list that jumps from 80 to 140 can each change the cake, the price, or whether you take the job.
Picture a baker in Pune who gets a WhatsApp saying "Need a 3 tier cake for December, how much?" She has to send six follow-up questions before she can answer. Put those questions in a form and the couple answers them once, in one sitting.
| Section | Fields to include | Why you need it |
|---|---|---|
| Contact | Both names, phone, email, best way to reach them | Either partner may reply, and many couples prefer WhatsApp to email |
| Date and venue | Wedding date, venue name and address, indoor or outdoor, reception start time | Heat, travel time and your baking schedule |
| Size and design | Guests served cake, tiers, colors, flowers or toppers, inspiration photos | Drives price and workload |
| Flavors and diet | Flavor per tier, allergies, eggless or vegan | Safety and ingredient planning |
| Tasting | Preferred slot from your list, number attending | Protects your calendar |
| Delivery | Delivery or pickup, venue contact, lift or stairs | Covered below |
| Budget | A range | Tells you which design to propose |
Ask for the guest count and how many will actually be served cake. Those are different numbers when there is also a dessert table, and your price depends on the second one.
Add a file upload field for inspiration photos. Couples already have screenshots on thier phones, and a photo saves three paragraphs of description. For budget, ask for a range instead of an exact figure. It tells you whether to suggest two tiers with fresh flowers or a five-tier fondant piece.
Tasting slots and deposit terms
For the tasting, list the two or three dates you really offer in a dropdown instead of asking couples to suggest their own. They pick from times you can work, and you skip the back and forth. If you charge for tastings, say the amount and whether it comes off the final bill.
The form cannot hold a live calendar. Qwenvo has no built-in scheduling, but you can embed Calendly with the Embed anything block if your tastihgs book up fast.
Put your deposit terms in a text block just above the submit button, in plain sentences. Say what the deposit is (30 percent of the quote, for example, if that suits you), that it holds the date, whether it is refundable, and when the balance is due. Follow it with a required checkbox: "I have read the deposit terms." You then have a record that the couple saw them before you quoted.
Collect the money after you send the quote, not on the first form. A couple asking for prices has not agreed to anything yet, and a payment step on an enquiry form asks for commitment before they have seen a number. When they accept, send a short second form with a payment block.
Delivery details that prevent a bad morning
Add one question: delivery or pickup. With conditional logic, the delivery questions only appear when the couple picks delivery, so pickup customers never see them.
For delivery, ask for the venue's address, the name and phone number of a person who will receive the cake on the day, and when you can set up. Ask where the cake table is, whether there are stairs or a working lift, and where you can park. Make that contact someone other than the bride or groom, who will be busy.
If you deliver yourself, this section also tells you how long the drive is before you agree to a fee.
Printable sheet or online form
For a printed version, lay out the same fields in Word or Google Docs on one or two pages. Leave lines for answers and put the deposit terms at the bottom with a signature line. It works at an expo table or your counter, where a couple will not pull out a phone to type.
The catch is that someone has to reetype the sheet at home. Hand couples a card with the link to the online form too. Qwenvo has no built-in QR code, but you can paste the link into any QR generator and print the result on the card.
Going the other way, you can export any online submission as a PDF and print it for the kitchen board.
Building it free, and the alternatives
I'd build the online version in Qwenvo, which our team makes. Start from the quote request template, or describe the form to the AI on the empty-form screen and edit what it gives you.
Several things that most form builders charge for are free there. You get no Qwenvo badge on your form, a custom domain such as orders.yourbakery.com, and partial submissions, so you can see where a couple stopped. You also get visit and drop-off analytics, unlimited teammates for a partner or assistant who answers enquiries, and AI triage that labels each submission by priority and drafts a suggested reply. The payment block charges a 0% Qwenvo platform fee, although Stripe's own fee still applies.
Tally is a fair alternative. Its free plan has unlimited forms and submissions, logic, payments, and file uploads up to 10 MB per file. Removing its branding, a custom domain, teammates and partial submissions are on Pro at $29 a month. Google Forms is free with a Google account and fine for a simple enquiry, but it has no built-in payments and limited design. Couples must also sign in with Google to upload photos, which is a real hurdle for a photo field.
Here is where Qwenvo falls short for a bker. Payments run through Stripe only, so UPI or bank transfer deposits cannot go through the payment block. Put those instructions in the confirmation email instead. Per-submission PDFs cannot draw Devanagari or Arabic text, so if couples write names in those scripts, use the CSV or XLSX export, which handles them. It also launched in October 2026, so there are few reviews yet.

Questions owners ask
Can I use only a printable form and skip the online one?
Yes, if every enquiry comes in person. But you wkll retype each sheet, and couples who find you on Instagram have nothing to fill in. Keep the printed sheet for events and give people the link as well.
Should I take the deposit on the first form?
Usually not. Send the quote first, then collect the deposit once the couple says yes. The first form's job is to get you the details for an accurate price.
How long is too long?
A wedding cakke form can run longer than a contact form, because the couple is about to spend real money. Split it into pages for contact and date, cake, tasting, and delivery, and turn on the progress bar. Partial submissions show you where couples stop, so you know which question to cut.
Do I need a website?
No. Paste the link into WhatsApp replies, your Instagram bio and your Google Business Profile. You can set a link preview image and title so it looks right when shared.
Start with one test order
Open the quote request template, delete the fields you do not need, then add a tasting dropdown and your deposit terms. Fill it in yourself on your phone, the way a couple would, and fix anything that confuses you. Then put the link in your Instagram bio and send it in your next WhatsApp enquiry reply.
